  Includes your team logo and team colors.
• CUSTOM OPTION 2 — $30
  Includes your team logo and team colors PLUS your athlete’s custom photo.

Payment: Your card on file with Kids First will be charged unless you request another payment method. If you would like to pay in person, by phone, or with a different card, please note your preference in the Comments section below.
